Trimethoprim is primarily used as an antibacterial agent. It has dihydrofolate reductase inhibitor activity with selectivity for the prokaryote enzyme. It is used to treat urinary tract infections, mild acute prostatitis, recurrent cystitis, asymptomatic bacteriuria during pregnancy and respiratory tract infections.
The combination of trimethoprim and sulfamethoxazole interferes with the cellular metabolism of folic acid in the bacterial cell by blocking the biosynthesis of nucleotides. Trimethoprim binds to dihydrofolate reductase and inhibits the reduction of dihydrofolic acid (DHF) to tetrahydrofolic acid (THF). THF is an essential precursor in the thymidine synthesis pathway and interference with this pathway inhibits bacterial DNA synthesis.

Description: Folate Receptor 1 (FOLR1) is also known as Folate receptor alpha, Folate Binding Protein (FBP), FOLR, and is a member of the folate receptor (FOLR) family. Members of this gene family have a high affinity for folic acid and for several reduced folic acid derivatives, and mediate delivery of 5-methyltetrahydrofolate to the interior of cells. Mature FOLR1 is an N-glycosylated protein that is anchored to the cell surface by a GPI linkage. FOLR1 is predominantly expressed on epithelial cells and is dramatically upregulated on many carcinomas. FOLR1 is internalized to the endosomal system where it dissociates from its ligand before recycling to the cell surface. A soluble form of FOLR1 can be proteolytically shed from the cell surface into the serum and breast milk. Defects in FOLR1 are the cause of neurodegeneration due to cerebral folate transport deficiency (NCFTD). NCFTD is an autosomal recessive disorder resulting from brain-specific folate deficiency early in life.

Sulfamethazine is an antimicrobial sulfur drug that blocks the synthesis of dihydrofolic acid by inhibiting the enzyme dihydropteroate synthase. Sulfamethazine is a competitive inhibitor of bacterial para-aminobenzoic acid (PABA), which is required for bacterial synthesis of folic acid. It induces CYP3A4 expression and is acetylated by N-acetyltransferase. It exhibits sex dependent pharmacokinetics, metabolized by the male specific isoform CYP2C11. Sulfamethazine is bacteriostatic.
